I will not be indexing the rulebook. I will put two levels of heading in
the contents.

This is because I don't use the index, and when I have tried it doesn't
have what I am looking for. I use the pdf and search.

If somebody (or somebodies) care enough to do the markup I can give them
access to the working document (docx) so that they can do it.
